Getting an advance for a structured settlement can be a life saver. A person who wins the settlement from an injury or wrongful death suit may be awarded a large sum of money; however, people who are awarded this large sum of money may never have managed this much money before; therefore, laws have been passed so that people who are not used to this large amount of money is structured to be dispersed over a period of time. This prevents people from going on a spending spree if the money is awarded in a lump sum. The funds that were structured were meant to pay a person?s medical bills over a lifetime but sometimes people have a financial emergency and need the money to pay off debt.

Structured settlements are guaranteed payments of funds paid to the claimant or his/her family on a tax-free basis. Unexpected takes on the familiar

I never understood why anyone would want to riff on the perfect, old-fashioned tuna fish salad -- until I did it my way.

Besides adding smoke flavor, grilling cabbage brings out the vegetable's natural sweetness. The result makes for an easy accompaniment to grilled meats, particularly pork. Or it can be the basis for a twist on your favorite coleslaw recipe. You'll need a grill basket.

Make ahead: You can make the salad a day in advance if necessary, but it's best the day it is made.

ROASTED RED PEPPER, TUNA AND BEAN SALAD

7 ounces solid white tuna packed in water, drained

1-3/4 cups cooked, no-salt-added Great Northern beans (one 15.5-ounce can, drained)

2 ounces sweet onion, such as Vidalia, cut into1/4-inch dice (1/3 cup)

1 medium rib celery, cut into1/4-inch dice (1/3 cup)

1/2 roasted red bell pepper, seeded and cut into1/4-inch dice (1/3 cup)

Juice of 2 lemons

2 tablespoons olive oil, or more to taste

3 tablespoons sherry vinegar, or more to taste

Salt

Freshly ground black pepper

Flake the tuna into medium-size pieces.

Combine the tuna, beans, onion, celery, roasted bell pepper, lemon juice, oil, vinegar and salt and pepper to taste in a large bowl.

Stir to combine.

Taste, and adjust the seasoning and/or oil as needed.

Makes 4 generous cups (4 to 6 servings).

GRILLED CABBAGE

3 to 4 tablespoons olive oil, plus more for the grill basket

1 medium green or red cabbage (about 2- 1/2 pounds)

Sea salt

Freshly ground black pepper

Prepare the grill for direct heat. If using a gas grill, preheat to medium-high (450 degrees) and add soaked wood chips in a foil packet or in a smoker box. If using a charcoal grill, light the charcoal or wood briquettes; when the briquettes are ready, distribute them under the cooking area for direct heat. For a medium-hot fire, you should be able to hold your hand about 6 inches above the coals for 4 or 5 seconds. Have ready a spray water bottle for taming any flames.

Lightly coat the grill rack with oil and place it on the grill.

Cut the cabbage in half. Cut each half into 1-inch-thick slabs, leaving the core intact. Brush both sides of each slab with the oil and sprinkle generously with salt, placing them in a grill basket as you work.

Grill the cabbage slabs until they are charred in spots on the outside and starting to wilt (but are not soft) on the inside, about 10 minutes per side. (If some of the leaves come loose and start to burn, use tongs to pull them off and transfer them to a plate while you cook the remaining cabbage.)

Transfer the cooked slabs to a cutting board; cut out and discard the core. Cut the grilled leaves into thin slices. If serving as a side dish, season with salt and pepper to taste.

First lady's book: On growing seeds, healthy kids

WASHINGTON (AP) ? From the beginning, Michelle Obama's kitchen garden has been an overachiever, churning out more peppers, parsley and eggplant than expected, and generating interest that ? yes, really ? crosses oceans.

Now, the first lady has added a 271-page book to her gardening resume, and Americans can read all about the planting misses that came with the hits, get tips on gardening at home, and, Mrs. Obama hopes, draw some inspiration that just might change their lives. Oh, and if it happens to help her husband's re-election campaign, that would be nice, too.

Lofty goals for a book about a garden.

In "American Grown: The Story of the White House Kitchen Garden and Gardens Across America," Mrs. Obama holds out the raised vegetable beds on the South Lawn as "an expression of my hopes" for the nation's children. "Just as each seed we plant has the potential to become something extraordinary, so does every child," she writes.

The $30 book, released Tuesday by Crown Publishers, traces how a city kid from the South Side of Chicago who became a working mother and then a political spouse found herself fretting on that first planting day, March 20, 2009, about whether an L-shaped stretch of soil would prove fertile ground for a national conversation "about the food we eat, the lives we lead, and how all of that affects our children."

The book, which answers that question with a resounding yes, arrives just in time for her husband's re-election campaign.

And while the book is decidedly non-political, that fits perfectly with the Obama campaign's view that the first lady can do her husband a world of good simply by pushing the non-threatening causes such as healthy living that have made her a far more popular figure than the president himself. Mrs. Obama's favorability rating in the latest AP-GfK poll was 70 percent, compared with 58 percent for her husband.

The book's release comes with a flurry of media appearances. She's already been a TV frequent flyer to promote her "Let's Move" campaign to combat childhood obesity ? doing pushups with Ellen DeGeneres, playing tug-of-war with Jimmy Fallon in the White House and serving veggie pizza to Jay Leno. She says she gets asked about the garden wherever she goes, around the world.

The book is chock full of colorful, glossy photos of luscious-looking vegetables, complete with a cover picture in which the first lady's blouse seems to be color-coordinated with the eggplants in her bulging basket of produce. Bo, the popular family dog, gets plenty of cameo appearances. There are maps tracing the growth of the garden over the past three years, and stories about community gardens around the country. Even a how-to on creating a compost bin.

The book is divided into four sections marking the seasons, and includes a complement of recipes for each.

There are inside stories about planting travails that will ring true with any weekend gardener: pumpkins that wouldn't grow, cantaloupes that tasted blah, blackberry bushes that wouldn't play nice with the raspberry bushes and an invasion of cucumber beetles, among them. The first lady makes clear she's not the one doing most of the hoeing and weeding, crediting school kids, White House chefs and grounds crew and enthusiastic volunteers from all over the White House chain of command with providing lots of manpower.

And there are bits of historical trivia woven throughout: John Adams ordered up the first White House garden, but it was never harvested after he lost re-election. Thomas Jefferson was obsessed with trying to grow a four-foot-long cucumber. Heiress Rachel "Bunny" Mellon, at 101 now a figure in the John Edwards' corruption trial, helped redesign the Rose Garden for President John Kennedy.

There are also bits of personal history: Mrs. Obama's father worked as a boy on one of the vegetable trucks that would deliver produce to neighborhoods, and had a reputation for sneaking pieces of fruit. Her mother's family had a plot in a neighborhood victory garden on the corner of an alley, and the kids had to eat their vegetables or go to bed without supper.

It is a tradition for first ladies to use books to advance their causes. Hillary Clinton wrote the best-seller "It Takes a Village," about the importance of community in raising children; and Laura Bush collaborated with daughter Jenna on a picture book about a reluctant reader, with "Read All About It!"

This is Michelle Obama's first book. She got no advance payment, and plans to donate all her proceeds to the National Park Foundation for programs that promote gardening and healthy eating, and to help care for the White House garden.

Richmond's Sahar Biniaz wins Miss Universe Canada

Richmond's Sahar Biniaz has been crowned Miss Universe Canada following a pageant in Toronto Saturday night.

The 26-year-old was born in India and raised in Iran before moving to Canada.

She graduated with honours from Stella Adler Academy in Los Angeles and is currently working as an actress in film and television.

Biniaz will now represent Canada at the 2012 Miss Universe competition in December. She was first runner-up in Miss Universe Canada 2008.

Said Donald Trump via Twitter (@realDonaldTrump): "Sahar Biniaz is beautiful. She will make Canada proud in the international @MissUniverse competition this December."

Biniaz played the role of Hawkgirl in the Lower Mainland shot TV series, Smallville, and was featured in the role of Kali in the season finale of locally-filmed Sanctuary, according to Wikipedia.org

Also entered in the competition were two other Richmond residents.

Activists waging a national war on coal have turned their sights on the Pacific Northwest, targeting proposed shipping terminals in Washington and Oregon that would export coal to?China.

They?re aggressively lobbying federal officials to change how these projects are evaluated. If they succeed, our economy could become a casualty of the war on?coal.

Currently, such projects undergo a rigorous environmental review known as an Environmental Impact Statement involving months or even years of public hearings and analysis by federal, state and county regulators. The EIS for each project must also examine the cumulative impacts of other potential activities in the?area.

But opponents want to insert a second, more expansive layer of environmental review, which some are calling a Programmatic EIS, which would have to be completed before each individual project EIS could begin. This additional review would include all of the Washington and Oregon proposals and expand to analyze their potential ?cumulative? economic and environmental impacts across the region, the United States or perhaps the?world.

A PEIS is historically reserved for assessing the broad national impacts of a federal action or a new federal policy. But the activists want to apply that same scope of review to a local shipping?terminal.

For example, what would be the collective impacts on air, water, wildlife and so on, if all the proposals are built? And what are the additional environmental impacts of the coal mines in Wyoming and Montana? What are the additional environmental impacts along the rail lines from the mines to the terminals? How about the environmental impacts of shipping the coal to Asia? Or the global impact of Asia burning U.S.?coal??

Opponents argue that all of these issues should be part of an additional environmental?review.

Couldn?t happen? Think?again.

Oregon Gov. John Kitzhaber, the Washington state Department of Ecology, the Region 10 office of the U.S. Environmental Protection Agency, the city of Seattle and the Yakama Nation are among those urging the U.S. Army Corps of Engineers to change the rules. They want the Corps to conduct a special analysis of the so-called cumulative effects of all the proposed?terminals.

Even if this extra layer of analysis didn?t ultimately block the projects, it would delay them for years. The opponents? presumed goal is to create delay and legal gridlock, making it so difficult, time-consuming and expensive that the backers ultimately give?up.

Call it ?death by a thousand?lawyers.?

Eric Johnson, executive director of the Washington Public Ports Association, agrees that these projects should undergo rigorous review. ?However,? he notes, ?this review should be of the project itself, not of the overall system of commerce across our region, the United States, or as urged by some commenters, the entire?world.?

Johnson warns that requiring an additional layer of analysis for these projects would set a dangerous precedent in a state where one of every three jobs is linked to trade. ?If this precedent is applied to all products imported and exported through our port transportation system, we will bog our project review timeline down in needless?process.?

Hi all,in my view,TK is lying about the device needing a ground ,however it is needed to at least provide a theory of why it appears that current is coming from the ground.Current coming from a ground wire in the manner of the TK video's is impossible,in my opinion.So I did a few experiments,in the greenbox video,ac voltage readings were impossible to read,if pulses from the induced first coil are mixed with ac current source you have this effect.The type of clamp amp meter,used in the video's I believe plays a part in false current amp readings,I have a meter that is close to the meter in the video's that's used in the experiments along with a different meter that doesn't react to current errors.A point on the ac voltage reading,all digital voltmeters
will have an error reading the ac voltage under the same conditions of? induced
pulses.On the frequency measured in the greenbox video that's inconclusive,I did another experiment,the meter will read the pulse frequency not the line frequency
this should apply to the greenbox video.A video shows that when a current source is removed,a false reading on the current meter disappears,then reappears when the current source reappears,reinforcing the belief that current thru the bulb is related to the ground current reading.

Our Trip to Donate to Our Local Animal Shelter #ILoveMyK9

If you've read this blog at all over the last two months, you know we adopted a dog (Roofus) from a local Humane Society Shelter. ?I was actually not too excited about adopting a shelter dog - I was leaning more toward buying a puppy from a breeder. ?I mistakenly thought that all of the dogs in shelters were either very old or problem dogs.


I couldn't have been more wrong!


Not only did we get an exceptionally loving and calm (as in really mellow) sweet dog under two years old, he came already potty trained and knowing a lot of basic commands such as sit and stay. ?Most of the other dogs in the shelter were puppies (so there goes that idea of old dogs - I think we only saw one or two over 4 years old) and none would be described as "problem" dogs. Our shelter even color codes them by personality so that you can get a dog that best matches your family's style.


I've always know the issues surrounding puppy mills and would never buy a dog from a pet store, but since our shelter experience, I have really come to respect what a wonderful service our local?Humane?Society Shelters offer. ?LJ has already announced that he'd like to volunteer there when he is old enough to be a dog walker.


When LJ and I were given the?opportunity?to donate some Del Monte dog treats to a worthy cause, we knew right away we wanted to give them to the shelter where we got Roofus. ?We were able to look up what they needed on their wish list and saw they were in need of treats for training and rewards. ?No wonder Roofus has such a love of Pup-Peroni, Milo's Homestyle Treats, and Milkbone treats - that what he got at the shelter!


LJ and I headed?off to Walmart to pick up some supplies?to donate to the shelter together.

LJ very carefully picked out the varieties of Pup-Peroni he thought the dogs would like best.

The Milo's Homestyle Treats stumped him a bit since there were so many difference choices!

He wound up grabbing one of each!

I had really wanted to make a nice cute gift basket, but we were able to get so many treats with Walmart's low prices that they wouldn't fit in the basket. Add in the two bags of dog food and we needed a box!!

The shelter we were headed to is about 40 minutes away, so we all loaded into the car and set off for our road trip.

We were so impressed that the ladies at the shelter remembered Roofus. ?

They were so happy to see him doing well!

It was a good thing that this little guy was already spoken for - LJ and I were both tempted to bring him home with us!!



I encourage you to look at your local shelter before purchasing a dog or puppy from somewhere else. There are so many beautiful, loving dogs who need a good home. If you are able to support your local shelter with a cash or goods donation, they will be most grateful for any help they receive!

Update on advanced prostate cancer - what are the new trends ...

This month has brought a flurry of regulatory activity in the prostate cancer landscape with several companies seeing noticeable action in the new product development area:

For this specific post, I want to concentrate on the new trends in advanced prostate cancer, as last weekend I attended the 2012?American Urological Association (AUA) annual meeting, focusing on the basic research sessions hosted by the Society of Basic Urological Research (SBUR) and Society of Urologic Oncology (SUO). ?It?s hard to ignore the clinical data though, with so much activity in going on in advanced prostate cancer!

While at the oral and poster sessions, I enjoyed informal chats with urologists and researchers. Many of the urologists I spoke to at AUA were very excited about enzalutamide, certainly much more enthusiastic than in the last two years when we saw the arrival of sipuleucel-T (Provenge) and abiraterone (Zytiga) in the pre- and post- chemo settings respectively. The reasons for this were varied, depending on the respondents, ranging from understanding the mechanism of action (MOA) clearly to ease of logistics to lack of concomitant steroid therapy. The fact that the initial overall survival of 4.8 months for enzalutamide in the post-chemo setting, which hints at a more potent and effective therapy, probably helped as well.

With that in mind, I spoke with Dr Neal Shore, one of the AFFIRM trialists about his perspective of the impact of enzalutamide on his patients. ?Firstly, we discussed the expectations for patients who have received chemotherapy:

?Patients who typically have completed a course of docetaxel have historically a 6-12 month life expectancy depending upon? on how symptomatic or asymptomatic they are.?

Secondly, what was the impact of treatment with enzalutamide?

?In the treatment arm, those who received enzalatumide, they lived at the median 4.8 months longer than patients who received the control placebo.

So in fact their survival extended beyond 18 months. I told you that normally this patient population has a 6-12 month survival expectancy range, that to me is dramatic.

It?s a dramatic life prolongation effect.?

One of the things that makes enzalutamide exciting for me is the ability to target splice variants. This may explain why the agent has slightly better efficacy in the post chemo setting than abiraterone, which demonstrated a 3.9 month overall survival advantage over placebo when it was initially approved. This figure has since improved to 4.7 months with more mature data, suggesting that the initial 4.8 months improvement we see with enzalutamide may also potentially improve further with time.

Another area where we may see changes in mCRPC is sequencing, combinations and different trial designs. While it seems logical that good old fashioned sequencing, as with ADTs in earlier disease, will prolong time to disease progression by managing PSA levels, it should be noted that I don?t think any of those agents have actually shown a significant improvement in overall survival in patients. Unlike oncology, urology tends to have looser endpoints and this is something that we may well see changes in going forward as more rigour is applied in the clinical trial setting.

Combination therapy is something that I think we will also see more of in the future if we are to see real shifts in meaningful outcomes. As Charles Sawyers noted in a previous interview, shutting down the AR pathway more comprehensively with dual inhibition and ?big guns? makes solid scientific sense. This doesn?t just mean the obvious though, such as determining whether dual upstream and downstream AR inhibition with enzalutamide plus abiraterone versus either alone would work, but also targeting cross-talk and adaptive resistance pathways such as AR inhibition plus a PI3K inhibitor. Some of these trials are already underway, at least in phase II to see what the safety and efficacy signals looks like.

The arrival of multiple new therapies that change the standard of care also means that once the current crop of new drugs have been approved (abiraterone, enzalutamide and alpharadin) it becomes more difficult to conduct placebo controlled trials with OS any more, as Dr Shore astutely observed:

Dr Shore?s point about surrogate markers of survival in advanced cancer is a highly relevant one given the increasing level of competition in this tumour type.

In the pre-chemotherapy setting, both abiraterone (302 trial) and enzalatumide (PREVAIL) appear to have progression-free survival (PFS) and OS as co-primary endpoints. It will be interesting to see how the FDA react if only PFS is significant while OS is not, as many commentators suspect from the rather vague and woolly press release that J&J put out for abiraterone. ?I don?t know what was agreed beforehand with the FDA regarding the study design, but I can see an interesting and highly charged ODAC ahead here as well as much speculation prior to the ASCO presentation of the data in the prostate cancer oral session next month.

OS is a clear, but challenging measure because ultimately, as Dr Shore noted, the endpoint is death. ?Remember, recent breast cancer trials discussed here on PSB achieved a significant OS of 6 months or more, precisely because crossover wasn?t allowed. These trials had an active arm as a comparator, not placebo, though. We don?t yet know if patients in the placebo arm who relapsed were allowed to crossover to abiraterone early (ie before the IDMC recommendation). If they did, then OS was doomed from the start and the trial design itself was flawed, but we will have to wait for the presentation to see before jumping to conclusions.

I really hope that?s not the case here, because once crossover does occur, it is very hard statistically, to sort out a true survival signal. If the placebo patients didn?t crossover before the IDMC recommended study stoppage, then it?s hard to see why OS wasn?t met (if truly the case) unless the patients had long term compliance and adherence problems as a result of the concomitant steroid therapy. I do find it hard to believe, however, that an IDMC would stop a trial early if a primary registration endpoint was not met, for that would be akin to regulatory hari-kiri!

Overall survival has long been the standard of care in advanced prostate cancer and several drugs have either received approval or will receive approval as a result of meeting that high hurdle. Future entrants will find it very hard to ethically justify using placebo in the comparator arm and will likely need to be compared to sipuleucel-T, docetaxel, cabazitaxel or abiraterone as the currently approved new standards of care and potentially other options will be considered that are seeking approval such alpharadin and enzalutamide in the near future.

This improvement in care raises the bar for companies considering advanced prostate cancer in several ways:

  • Increased costs (active comparator arm, prolongation of treatment)
  • Increased time to market (OS takes longer than PFS)

In the end, though, for men with prostate cancer it?s largely all good news as new therapies that clearly prolong life become available and make a difference to their lives, not just in terms of more time with their families, but also in terms of improved quality of life and symptom management.
